Please call our office at (425) 455-0936. You may also schedule an appointment online. If you are unable to keep your appointment, please inform us as soon as possible so that we may use that time to see another patient in need of care. If you cancel less than 24 hours in advance or no show to your appointment, there will be a $40.00 fee.
We are currently accepting new patients. Please arrive 15 minutes prior to your appointment time to complete the registration process. You may also download and fill out the new patient forms prior to your visit. Please bring the following to your appointment:
- New Patient Forms
- Driver's license or state ID
- Insurance cards
- List of allergies and current medications
- Any relevant previous medical records
We require payment for all services at the time they are rendered. We accept cash, checks, debit, Visa, MasterCard, and American Express. If we are an in-network provider, we will bill your insurance company. However, payment is the patient's responsibility.
You may make a payment by calling our office during normal business hours. You may also pay online. For all billing questions, please call our billing office at 877-385-0257.
